Fan-O Flavor Form

Dan-O's Seasoning, LLC, on behalf of itself, its partner and affiliate companies, subsidiaries, dba companies, and associated clients, (“The Companies”) participants may be photographed during companies' activities, planned or unplanned photo or video events. I, the undersigned, hereby grant The Companies' permission to the rights of my image, likeness, and sound of my voice in any video, audio and photography taken or recorded of me by The Companies. This authorization extends to the right to edit, alter, copy, exhibit, publish and distribute any and all video, audio, and photography taken of me by The Companies.

The Companies' rights include but are not limited to, the rights, in perpetuity, to:
Use, re-use, publish, and re-publish the content; alter, modify or otherwise change the content in any manner The Companies desire; combine the content with textual matter and/or with other pictures and/or media; and, use the content for illustration, promotion, art, editorial, advertising, trade, publishing, client or partner websites or any other purpose whatsoever. I also understand that once my image is posted on the internet, the image can be downloaded.

This authorization extends to all languages, media, formats, and markets known or later discovered.

I waive the right to inspect or approve any finished product in which my likeness appears.

I agreed to this release without being compensated. I waive any right to royalties or other compensation arising or related to the use of the media.

I understand and agree that these materials shall become the property of The Companies and will not be returned.

I hereby warrant that I am of full legal age and have the right to contract in my own name. I have read the above authorization, release, and agreement, prior to its execution, and I am fully familiar with the contents thereof. This release shall be binding upon me and my heirs, legal representatives and assigns.
